19 SUBCUTANEOUS IMPLANTABLE CARDIOVERTER DEFIBRILLATOR AFTER TRANSVENOUS LEAD EXTRACTION: SAFETY, EFFICACY AND OUTCOME

Abstract Background Subcutaneous implantable cardioverter defibrillator (S-ICD) is a suitable alternative for transvenous ICD (TV-ICD) patients who have undergone lead extraction (TLE). Limited data are available on the outcome of S-ICD implanted after TLE. We assessed safety, efficacy and implantation TLE TV-ICD. Methods The study population consisted 36 consecutive with median age 52 (44-66) years underwent Results Conclusions Indications were: infection (63.4%) malfunction (36.4%). During follow-up 31 months, 3 (8.3%) experienced appropriate therapy 7 (19.4%) complications including inappropriate (n=4;11.1%), pulse generator pocket erosion no evidence (n=2;5.5%) ineffective (n=1;2.7%).No or systemic device-related infection, lead/hardware dysfunction were reported.Premature device explantation occurred in 4 (11%). Eight (22.2%) died during follow-up, six them (75%) because refractory heart failure (HF).There S-ICD-related deaths. Predictors mortality included NYHA class ≥2 (HR 5.05;95% CI 1.00–26.38;p=0.04), hypertension 22.72;95% 1.05–26.31;p=0.02), diabetes 10.64;95% 2.05-55.60; p=0.001) ischemic disease 5.92;95% 1.17–30.30;p= 0.01). Our provides evidences use as an ideal TV-ICD both failure. Mortality does not appear to be correlated presence prior (appropriate inappropriate), but rather worsening HF other comorbidity.
